QIIP - Quality Improvement & Innovation Partnership

The Quality Improvement and Innovation Partnership project which serves all provincial Family Health Teams, is working with a consulting firm, CSI Solutions, to embark on a series of initiatives designed to address three main areas for improvements in provincial health delivery and management.  These are:  Access and Efficiency, Diabetes Management and Colorectal Cancer Screening.

The Centre for Family Medicine joined the first Learning Collaborative in April of 2008 and in May attended the first Learning Session.

We were introduced to a new model of care that focuses on changing practice and moving towards the ideal state of "planned care". The Ontario Chronic Disease Prevention and Management Framework model.
