The Diabetes Clinic is designed to help CFFM patients manage their blood sugars and to lower their risk of diabetic complications.  The Clinic operates on Tuesday mornings at the 25 Joseph Street site and is staffed by a chronic disease management educator, clinical pharmacist and clinical dietitian.  You can ask your health care provider for a referral.
